The short version
TotalRx is our top pick because its promises held up when we read the fine print. You get one monthly price with no membership on top, it does not change while your plan is active, and you get your money back if a clinician says no. It is not the cheapest program we checked, and it only offers compounded injections.

Side effects and safety
Like all GLP-1s, the common side effects are stomach-related: nausea, constipation, diarrhea and sometimes vomiting, mostly when the dose goes up. GLP-1s are not for everyone, including people who are pregnant or have a personal or family history of medullary thyroid cancer. The clinician review is where that gets checked.
