Online GLP-1 Programs, Checked: What You Really Pay in 2026
Most GLP-1 ads show you the first payment. We look at the rest: the monthly price after that, membership fees, what happens when your dose goes up, and whether you get money back if you stop. Then we score every program the same way.

What we found this month
- The cheapest sticker price isn’t always the cheapest year. Flat-rate compounded programs ran from $69 to $129 a month. Brand-name programs started at $149 but added a membership of up to $149 a month.
- Refund rules vary a lot. One low-cost program treats every payment as final. Our top pick refunds you in full if a clinician says no.
- Some clinics only show month one. If a program won’t tell you month two before checkout, it loses points in our transparency score.

How we score programs
Seven factors, weighted the same way for every program. Payment from a provider never changes a score. Full method.
GLP-1s in plain English
GLP-1 medicines act like a hormone your gut makes after a meal. They turn down hunger, help you feel full on less food and slow how fast your stomach empties. Semaglutide is the ingredient in Wegovy® and Ozempic®. Tirzepatide is the ingredient in Zepbound® and Mounjaro®, and it also acts on a second hormone, GIP.
In large trials of the brand-name drugs, people lost on average about 15% of body weight on semaglutide 2.4 mg over 68 weeks (STEP 1) and about 20% on higher doses of tirzepatide over 72 weeks (SURMOUNT-1), alongside diet and activity changes. Your results may differ.
Brand-name or compounded?
Brand-name versions are FDA-approved and made by Novo Nordisk or Eli Lilly. Compounded versions are mixed by licensed pharmacies and usually cost less, but they are not FDA-approved and the FDA does not check them for safety, effectiveness or quality. If you go compounded, ask which pharmacy makes it.
